Username Recovery and Password Reset

Have you forgotten the username or password to your Ontario Health Study account? These can be quickly recovered or reset from the Sign in page. Please see below for detailed instructions.

How to recover your username

  1. Click Forgot your username?
  2. You will then see the following page:
  3. Please enter the email address you used to register with for the Study. If you have updated your contact information, please enter the most recent email address you have associated with your Ontario Health Study account.
  4. Click Send me my Username.
  5. You will be sent an email (to the email on file and confirmed above) containing your username.
  6. When you close this pop-up, you should return to the log in page to log into your account with your correct username.

How to reset your password via email

  1. From the login page enter your correct username. If the correct username is not used, password recovery will not work.
  2. Click Forgot your password?
  3. You will see a popup asking you to choose the method for resetting your password.
  4. Click on the first option that says Click here to reset your password via email.
  5. You will then see the following page appear:
  6. Enter your username or the email address you used to register for the Study in the grey box. If you have updated your contact information, please enter the most recent email address associated with your Ontario Health Study account.
  7. Click Send me my password reset email.
  8. If the correct username or email address was entered, you will receive a message stating that the password reset link has been sent to the email address associated with your Ontario Health Study account. If an incorrect username or email address was entered, or if the email address is associated with multiple accounts, an error message will appear. If this occurs, please contact a Study Team member for assistance using the contact information provided.
  9. Please open your email to check for the email with the password reset link. For security reasons, the password reset link is only valid for 30 minutes. Click the link in the email and a new box will appear asking you to create a new password.
  10. You will then be prompted to create a new password and confirm it.
  11. Once you have entered and confirmed your new password click on Apply.
  12. After successfully entering a new password you will be taken back to the main login page to log back into the study with your newly created password.
  13. This password reset method will also unlock a locked account.

How to reset your password via security questions

  1. From the login page enter your correct username. If the correct user name is not used the password recovery will not work.
  2. Click Forgot your password?
  3. You will see a popup asking you to choose the method for resetting your password.
  4. Click on the second option that says Click here to reset your password using security questions. Only select this option if you recall answering security questions when you created your account.
  5.  The spam guard will then appear:
  6. Please enter the letters you see in the box at the right hand side, in the empty box on the left hand side and click Enter.

The letters are not case sensitive and do not contain numbers. If you see a capital letter you can enter it as a non-capital letter (lower case letter).

If you are unsure of the letters shown, you may refresh the letters yourself (click on the word Refresh, to the right of the box displaying the spam guard letters) until you see letters presented that you believe you will be successful in entering in the empty box at the left.

If the letters shown on the right refresh themselves, this means what you have entered did not match or the time allowed to enter the letters has expired.

  1. Please continue to enter the letters until you are successful.
  2. Once you are successful you will be prompted to answer your security questions.
  3. Once you have answered your security questions correctly click OK.
  4. You will then be prompted to create a new password and confirm it.
  5. Once you have confirmed your new password click Apply.
  6. After successfully creating a new password you will be taken back to the main login page to log back into the study with your newly created password.
  7. This password reset method will also unlock a locked account. 

Tips for username recovery and password reset

  • If you have forgotten both your username and your password, you must recover your username first before resetting your password.
  • The recovery and reset tools require a unique email address in our system. If you have multiple accounts associated with the same email address, please contact a Study representative for assistance.
  • Please note that the spam guard is NOT case sensitive, so please ensure that you are entering all the letters in lowercase, with no spaces. Once you enter all the letters in lower-case with no spaces, please hit the "Tab" button on your keyboard. Hitting the Tab button on your keyboard instead of clicking on the "Enter" beside the grey box will let you get past the spam guard if clicking “Enter” does not work.
  • As a security precaution, password reset links sent to your email address are only valid for 30 minutes. If this time expires before it is used, a new link must be sent.
  • If your account has been locked due to repeated unsuccessful attempts to log in, it can be unlocked by resetting your password.
